ORDINANCE NO. 961228

Authorizing a nine month $33,000.00 agreement with Michael Enochs to provide programming support for the Kansas City Immunization Information System; establishing a revenue account; and appropriating $10,000.00 to the Information Systems account.

BE IT ORDAINED BY THE COUNCIL OF KANSAS CITY:

Section 1. That an agreement between the Kansas City, Missouri Health Department, acting through its Director of Health, and Michael Enochs, an individual, whereby Enochs will provide programming support for the Kansas City Immunization Information System (KCIIS) for the period of August 1, 1996 through April 30, 1997, for which the City will provide funding in an amount not to exceed $33,000.00 from funds previously appropriated in Account No. 97-100-050-1020-B, is hereby authorized. A copy of the agreement, in substantial form, is attached hereto and made a part hereof by reference.

Section 2. That the following revenue account is hereby established in the General Fund in the following amount:

97-100-050-8332 Contribution from MAIC $10,000.00

Section 3. That the following appropriation account of the General Fund is hereby increased in the following amount:

97-100-050-1020-B Information Systems $10,000.00

I hereby certify that there is a balance, otherwise unencumbered, to the credit of the appropriation to which the foregoing expenditure is to be charged, and a cash balance, otherwise unencumbered, in the treasury, to the credit of the fund from which payment is to be made, each sufficient to meet the obligation hereby incurred.
