ORDINANCE NO. 950636

 

Accepting and approving an agreement with the Department of Health to provide services to reduce the burden of Diabetes Mellitus for an eight month period for which the State will provide $33,766.00 in funds; establishing a revenue account and appropriating said sum in the General Fund; accepting and approving a contract with the East Meyer Neighborhood Association to provide these services; and retaining $983.00 in administrative costs.

 

BE IT ORDAINED BY THE COUNCIL OF KANSAS CITY:

 

Section 1. That a contract between the Kansas City, Missouri Health Department, acting through its Director of Health, and the Missouri Department of Health, acting through its Director of Health, to provide services to reduce the burden of Diabetes Mellitus and increase the daily fruit and vegetable intake of adults in Kansas City, Missouri, who are food stamp recipients for the period of February 1, 1995 through September 30, 1995, for which the State will provide funding in the amount of $33,766.00, is hereby accepted and approved. A copy of the contract is attached hereto and made a part hereof by reference.

 

Section 2. That the following revenue account is hereby established in the General Fund and the same is hereby estimated in the following amount:

 

96-100-050-7317 95 Diabetes Today $33,766.00

 

Section 3. That the following appropriation account of the General Fund is hereby increased in the following amount:

 

96-100-050-2330-B Hypertension 96 $33,766.00

 

Section 4. That an agreement between the Kansas City, Missouri Health Department, acting through its Director of Health, and East Meyer Neighborhood Association, acting through its Executive Director, to provide services to reduce the burden of Diabetes Mellitus and increase the daily fruit and vegetable intake of adults in Kansas City, Missouri, who are food stamp recipients for the period of February 1, 1995 through September 30, 1995, for which the City will provide $32,783.00 in funds herein appropriated in Account No. 96-100-050-2330-B, is hereby accepted and approved. A copy of the agreement is attached hereto and made a part hereof by reference.

 

Section 5. That the amount of $983.00, as specified in the agreement, may be retained by the Health Department to partially offset the cost of administering the agreement and may be used in such manner and for such purposes as the Director of Health may determine.

I hereby certify that there is a balance, otherwise unencumbered, to the credit of the appropriation to which the foregoing expenditure is to be charged, and a cash balance, otherwise unencumbered, in the treasury, to the credit of the fund from which payment is to be made, each sufficient to meet the obligation hereby incurred.
